4. Depends entirely on the insurance plan you are under. If your card covers mental health costs and a mental dr is willing to sign off that it is medically necessary for you, then your chances would be higher.
5. It should cover male breast reduction because male breast growth is a medical condition/abnormality. It probably will not cover tummy reduction unless it causes a medical problem, ie: skin rash, otherwise uncontrollable.
